GLP-1 receptor agonist medications, known by brand names like Zepbound and Wegovy, were initially developed to treat type 2 diabetes. However, their ability to regulate appetite and support weight reduction has led to their growing use as an effective option for long-term weight management.
The question many ask is: how much weight can I realistically lose on semaglutide or tirzepatide?
Let’s cut through the hype and take a grounded look at clinical results, individual variability, and what to expect.
GLP-1 Clinical Trials Show Promising Reduction in Body Weight
The most cited GLP-1 study is the STEP 1 trial, which evaluated semaglutide, the active ingredient in Wegovy, in non-diabetic adults with obesity. Participants who took 2.4 mg of semaglutide weekly lost an average of 14.9% of their body weight over 68 weeks (about 16 months), compared to 2.4% in the placebo group.
Tirzepatide, the active ingredient in Zepbound, was found to have similar effects during the SURMOUNT-1 trial. Non-diabetic patients who received 15mg weekly for 72 weeks
(about 17 months) lost an average of 22.5% of their body weight, compared to 2.4% of the placebo group.
Factors That Influence Weight Loss Results While Taking a GLP-1
While clinical data gives a strong baseline, individual results can vary based on several factors:
- Dosage & Duration: Not everyone starts at or reaches the full dose of a GLP-1 medication right away. Titration and tolerance affect the pace of results.
- Lifestyle Changes: Diet quality, calorie intake, and exercise still play a critical role.
- Medical Conditions: Conditions like PCOS, insulin resistance, or thyroid disorders can influence outcomes.
- Consistency: Regular injections and follow-up appointments with a healthcare provider can improve adherence and increase success rates.
Realistic Weight Loss from GLP-1 Injections
With weight loss stories circulating on social media and influencers touting dramatic results, it’s easy to assume that GLP-1 medications guarantee rapid, massive transformations. But while these treatments are clinically effective, real-world outcomes often tell a more nuanced story—especially when guided by medical professionals who monitor patients over time.
Realistically, many users experience:
- 5–10 pounds lost in the first month, primarily due to reduced appetite and calorie intake
- 10–20% total body weight loss over 6–12 months (depending on the drug)
- Slower, steady progress as the body adjusts to the medication
- Sustained weight loss is often achievable with continued use of the medication after reaching your goal weight
To shed light on what’s truly typical, we asked a provider with firsthand experience.
Claudia Guepet, FNP-C, a provider at Rejuvime Medical in Metairie, Louisiana, specializes in medically supervised weight loss and has guided many patients through GLP–1–based treatment plans. When asked if these medications truly lived up to the hype, this is what she had to say:
The results of medications like Semaglutide or Tirzepatide vary from person to person, as each person’s body composition and metabolism are different. It’s important to remember that everyone’s weight loss journey is different and shouldn’t be compared to someone else’s. A healthy and realistic weekly weight loss on Semaglutide averages about 1-2 pounds per week, dependent on other factors such as diet and physical activity level, which is highly encouraged. We will typically see healthy weight loss averages anywhere between 10-25 pounds (about 5-10% of body weight) in 3 months’ time. Some patients may lose over 30 pounds in 3 months, but we worry about the caloric deficit, muscle loss, nutrient deficiencies, and health risks that come with rapid weight loss. We encourage visiting a nutritionist to make sure your caloric needs are being met while taking a GLP-1.
Achieving Lasting Results from a GLP-1
GLP-1 medications are among the most effective weight-loss tools currently available. Significant reduction in body weight can improve health outcomes and quality of life for many patients. But results take time, consistency, and a realistic mindset. These medications are powerful tools, not magic solutions.
If considering the use of a GLP-1, set realistic expectations and maximize their effectiveness with these tips:
- Medications like semaglutide or tirzepatide deliver clinically meaningful weight loss over time. Reduction in body weight is gradual, not instant.
- Combine medication with a healthy diet and regular exercise for optimal results.
- Stay consistent and work with a healthcare provider to adjust dosing and track progress.
- Ongoing use may be needed to maintain weight loss, as stopping the medication often leads to gradual weight regain, especially in the absence of lifestyle changes.
